VOLUNTEER WORK AND DONATED RESOURCES FORM

Road/Debris Cleanup on PUBLIC PROPERTY as a Direct Result of Winter Storm Fern

It is very important that Lewis County documents all of the Volunteer Work that was performed, and all of the Equipment that was used, on Public Property during Winter Storm Fern (including county roads and rights-of-way).

This information could help Lewis County with part of the local, cost-sharing obligation that will be required by FEMA in matching funds or resources IF a federal declaration of disaster is filed for our community.

If you or your business volunteered your time, and/or equipment, to help clean up Public Property during Winter Storm Fern, please fill out the form below. By doing this, you will once again be helping out your community!

There is also a downloadable version of the form here if you'd rather turn it in in person. Click below to download!
